Passports

The Town Clerk’s office is an approved Passport Acceptance Facility.

You will need to complete, BUT NOT SIGN, a passport application (available in Town Clerk’s office or www.travel.state.gov), two identical, current passport photographs and a certified birth certificate with a raised seal AND date of record (caution – not all birth certificates contain the date of record). Your birth certificate must accompany the passport application but is returned to you with your passport. You may qualify for a renewal as well – call for details.

All applicants must be present to apply.  Effective February 1, 2008, the appearance and consent of both parents is required for minors under the age of 16.  A notorized statement of consent is required if one of the parents cannot be present.  The forms are available in the Town Clerk's office or at www.travel.state.gov

Allow 6 weeks to receive your passport and check the above website for updated travel information and guidelines.
